Life Care Planning by a Certified Legal Nurse Expert
A Life Care Plan is a dynamic document based on published standards of practice.
A Life Care Plan is written for individuals who have experienced catastrophic injury or who have chronic care needs. It outlines the annual and life time costs for long-term care for the life expectancy. The Life Care Plan is a working document which can be updated and revised as the individual progresses through acute to long-term rehabilitation. A Life Care Plans is a comprehensive compilation of a client’s medical, educational, rehabilitative, psychological, vocational, adaptive and supportive care needs and costs.
Life Care Plans are generally used by attorneys for settlement or trial purposes. Insurance companies can also use this type of research. These plans can be used as part of the assistance offered by the federal government with Medicare Set Asides and Workers’ Compensation. Families caring for their elderly utilize Life Care Plans to assist in caring for their loved ones.
The typical Life Care Plan covers areas such as evaluations; ongoing therapies; diagnostic, educational, and vocational needs; medications; supplies; orthotics and prosthetics; wheelchair needs; wheelchair accessories and maintenance; aids for independent functioning; home furnishings and accessories; home and facility care; future routine medical care; transportation; health and strength; leisure; architectural renovations; future medical care, including surgery and therapy; orthopedic equipment needs; and complications. This framework of services addresses comprehensive medical needs for the client, counseling for immediate family, and non-medical issues such as home modifications and vehicle adaptations.
Clients using the services of a Life Care Planner include those with spinal cord injuries, burn injuries, amputation, birth trauma injuries, transplantation, HIV/AIDS, and chronically ill or chronic pain patients. As our society ages, many Life Care Planners are being used by family members to plan a lifetime of care, services and products for our elderly.
To develop the plan, the Life Care Planner reviews the medical records; interviews the client and his family/caregivers; communicates with the medical, rehabilitative, vocational, and psychological team; performs medical research; researches equipment and vendors; and determines costs. The Life Care Planner then applies these findings to determine duration, frequency and future probability of the patients’ needs through their life expectancy as established by the principal treating physician. Once the life care plan has been completed, an economist converts its cost to present day dollars.
